Swing Orthopedic & Sports Physical Therapy will treat patients that were involved in a Motor Vehicle Accident (MVA) or have sustained injuries from an accident where a third-party is involved. MVA patients have the following options:

• Provide their private health insurance information. All copays, coinsurances and deductibles must be paid at time of service according to our contract with the health insurance plan provided
• Pay a self-pay rate at the time of service for each visit
• Defer payment with a Medical Lien. Patients who would like to defer payment must complete the Medical Lien Form. The Medical LienForm must be completed in its entirety prior to the initial appointment. If the paperwork is not completed, the patient will not be able to be seen. The patient is ultimately responsible for all costs related to medical care. If a Medical Lien is filed, anything that is not paid out by the lien will be the responsibility of the patient.

Once the utilization of one of the above options is elected, patient will not be able to switch to another billing option during the entirety of their treatment.
